In order to make an appointment for a consultation with Dr Imani you will need the following:

1. A referral from your General Practitioner or a GP of your choice, another specialist medical practitioner; in person appointment is preferred however if you prefer a Telehealth video consult is acceptable for the purposes of obtaining a GP referral.

2. A second consult with Dr Imani prior to deciding on surgery (this can be via a telehealth video call) 

3. Fill out questionnaires to ascertain the need for further referral to a Gp, psychologist or psychiatrist.
